You may fill out our 3-page Patient Form by clicking on the button above. BEFORE YOU FILL OUT the form, please download it to your computer. (If you fill it out online, your changes will not be saved in the download.)

After downloading, you may interactively fill out the form with typed text. When completed, save it and email it to us at Patients@2020view.com, or print it out and bring the completed pages to your appointment. If you don't have time to complete the forms in advance, we will give you the forms to fill out during your appointment.

We appreciate those who fill out the form in advance. Please arrive at your appointment 20 minutes early if you have not filled out this form yet. Otherwise, please arrive 10 minutes early.

Don’t forget to bring any insurance, Medicare and/or Medicaid card(s) with you.​

As our patient, you may pay-as-you-go with cash, check or credit cards – or use your medical or vision insurance, Medicare, Medicaid – or apply for CareCredit, which allows payments for up to two years at 0% interest!

Care Credit

Convenient Payments with CareCredit

CareCredit offers financial credit to qualifying patients to pay for eye exams, eyeglasses, LASIK, cataract surgery and certain other eye care services.

Patients of Eye Associates of the South may receive credit terms of 0% (zero percent) interest with up to 2 years to pay (based on services rendered) for our included optical goods and services.

Insurance: Vision vs. Medical

As our patient, you may pay-as-you-go with cash, check or credit cards – or use your medical or vision insurance, Medicare, Medicaid – or apply for CareCredit, which allows payments for up to two years at 0% interest!

Vision Insurance

Vision insurance is for routine eye exams. For example, it may pay if you are ready for new eyeglasses or you just can’t see as well as you used to. You visit Eye Associates of the South for an eye exam and receive a prescription for new glasses from the eye doctor. That would be covered by vision insurance such as VSP, EyeMed, Always Care, CompBenefits, etc. These insurances pay for the exam and have allowances for eyeglasses and contact lenses.

Medical Insurance

Medical Insurance could cover all other, non-routine visits to Eye Associates of the South. This may include a wide range of services from our doctors – such as cataracts, glaucoma, dry eyes, headaches, double vision, crossed eyes, etc.

We accept most types of insurance, including the list below. If you have a different insurance plan, please call us to see if we accept your policy. We often update our insurance list.
